The nurse navigator program is a resident assistance program to help residents obtain the medical care they need.

The Nurse Navigator Program at Park Central assists residents in obtaining the medical care they need to stay as healthy and independent as possible. The Wellness Liaison provides education and wellness services such as Forever Fitness, Living My Best Life, and health educations on campus and in the community. These services are offered at no additional charge and are just some of the many benefits of living at Park Central!

What is the Nurse Navigator Program?

The Nurse Navigator Program at Park Central is unique to senior living facilities. This program gives family members of those in senior living communities peace of mind that their loved ones are being taken care of. The Nurse Navigator Program provides someone to help you or your loved one understand some of the terminologies that go with insurance, services you can get from different providers, and helping residents get the services and level of care they need to remain as independent as possible.

Does the Nurse Navigator respond to life alert situations?

The Park Central Nurse Navigators are here seven days a week available to assist in emergency situations.  The resident will be assisted if possible, or the Nurse Navigator will call for an ambulance. Our Nurse Navigator will stay with the resident and provide basic support for them until the ambulance or fire department gets there.

Does the Nurse Navigator coordinate with the clinic on campus?

If a resident is ill or not feeling well, we have a clinic here on the Park Central campus. The Nurse Navigator can help residents get to the clinic, or if they’re too ill to go to the clinic, then a home visit can be coordinated with telemedicine visits or face time visits in the apartments.
